Eligibility
To be eligible for ADHC, individuals must be 18 years of age or older and
Physician-prescribed rehabilitative services for a medical condition.
Mental or physical impairments which handicap daily living activities.
Likelihood that individual's level of functioning can either stabilize or improve
Condition is likely to deteriorate, leading to institutionalization without ADHC services
(Title 22, California Code of Regulations, Sections 54209)
